1,353. Cerebellopontine angle tumor (schwannoma)

ชาย 63 ปี เวียนศรีษะ เดินเซ หูขวาไม่ค่อยได้ยิน ประมาณ 2 ปี ไม่มีประวัติบาดเจ็บศรีษะ ตรวจพบมี Wide base gait, impaired tandem walk,  impaired hearing of right ear

ผล CT brain พบมี Isodensity mass with heterogeneous enhancement at right cerebellopontine angle (CPA) possibly acoustic schwannoma

Cerebellopontine angle tumor เป็นเนื้องอกที่พบบ่อยที่สุดใน posterior fossa ประมาณ 5-10% ของเนื้องอกในสมอง โดยส่วนใหญ่เป็น benign
เนื้องอกบริเวณ cerebellopontine angle เช่น acoustic neuroma (vestibular schwannomas), meningioma, neurofibromatosis type II
ทำให้เกิดอาการเวียนศีรษะได้ acoustic neuroma เป็น cerebellopontine angle tumor ที่พบบ่อยที่สุด(พบได้ถึง 85%) มักเกิดจาก vestibular portion of CN VIII ทำให้มี vertigo, hearing loss, facial sensory loss (CN V), facial weakness (CN VII) ได้
