จากตารางด้านล่างถ้าผู้ป่วยมีภาวะน้ำตาลต่ำจริง และระดับ insulin น้อยกว่า 6 µIU/ml ให้ W/U เรื่องเนื้องอก ส่วนระดับ Morning cortisol ยังไม่ต่ำอย่างชัดเจน (low normal) ดังนั้นจึงยังไม่สามารถทราบสาเหตุที่ชัดเจนในผู้ป่วย คงต้อง W/U เรื่องเนื้องอกต่อครับ เพราะทำให้เกิดการเพิ่มการใช้กลูโคสและลดการปล่อยกลูโคสออกมา เช่น tumors occur primarily in the mesenchyma, liver, adrenal cortex, GI system และ lymphatic system โดยอาจจะเป็น benign หรือ malignant ก็ได้
ให้ R/o Postprandial Hypoglycemia ออกก่อน
ตอบลบน่าจะพอบอกได้ว่า เกิดจาก Cortisol insufficiency
ให้หาสาเหตุเรื่องนี้และ แก้ไขตามสาเหตุ ถ้าไม่พบสาเหตุก็ให้ Hormone Replacement RX
Ddx อื่น ๆ คือ Huge Cancer e.g. Huge Liver cancer
(e.g., hepatomas, adrenocortical carcinomas, carcinoids).
