ชาย 57 ปี HT, อ่อนเพลีย เวียนศรีษะ ตรวจพบมีหน้าแดง ม้ามโตเล็กน้อย CBC ดังข้างล่าง, O2 sat room air 90%, ได้ออกซิเจน cannula เพิ่มเป็น 93%, คิดว่าเป็น Polycythemia vera ได้หรือไม่อย่างไร, หรือมีแนวทางในการวินิจฉัยอย่างไร

WHO criteria for diagnosis of polycythemia vera (ล่าสุด)
Major criteria
1. Evidence of increased RBC volume, as least one of the following:
-Hb more than 18.5 g/dL in men or more than16.5 g/dL in women
-Hb or Hct more than 99th percentile of method-specific reference range for age, sex, and altitude of residence
-Hb more than 17 g/dL in men or 15 g/dL in women if associated with a documented and sustained increase of at least 2 g/dL from the patient's baseline value not accounted for by correction of iron deficiency
-Elevated RBC mass more than 25% above mean normal predicted value
2. Presence of JAK2 617VF or other functionally similar mutation (eg, JAK2 exon 12 mutation)
Minor criteria
1. Bone marrow biopsy showing hypercellularity for age with trilineage growth (panmyelosis) and prominent erythroid, granulocytic, and megakaryocytic proliferation
2. Serum erythropoietin level below the reference range for normal
3. Endogenous erythroid colony formation in vitro
การวินิจฉัยโดยมี 2 major criteria ร่วมกับ 1 minor criteria หรือ 1 major criteria ร่วมกับ 2 minor criteria
ดังนั้นจะพบว่าผู้ป่วยมี 1 major criteria แน่ๆ คือการที่มี Hb มากกว่า 18.5 g/dL ส่วน JAK2 617VF or other functionally similar mutation และ criteria ของ minor criteria คงต้องตรวจเพิ่มเติม
