Radial nerve palsy
Symptoms
-Numbness and a reduction of felt sensations on the back surface of the forearm and hand
-Tingling sensation or pain of the affected upper extremity
-Wirst drop -- the patient cannot extend the wrist and fingers outward when the palm is in a downward position
Cause
-Trauma: fracture
-Nerve compression by an anatomical structure surrounding the radial nerve
-External compression of the radial nerve -- after sleeping with an arm over the back of a chair
Treatment
ส่วนใหญ่หายเอง แต่อาจจะหายสนิทหรือบางส่วนรวมทั้งขึ้นกับสาเหตุและความรุนแรง
-Conservative treatment -- physical therapy, wrist splint or brace
-Surgical decompression ในกรณีที่เป็นนานและมีสิ่งที่มากดเบียด
-Surgical exploration ในกรณีที่หลายเดือนแล้วยังไม่หายเป็นปกติโดยเฉพาะจากการมีกระดูกหัก
ในผู้ป่วยรายนี้ที่แปลกคือไม่ได้ประวัติที่จะเป็นสาเหตุเลย (โดยผู้ป่วยก็ไม่ได้นอนหลับแล้วตื่นขึ้นมาเกิดอาการ) ซึ่งพบได้น้อย แต่ก็ควรหาสาเหตุก่อน เช่น neuritis, tumors หรือ compression

ถ้ามีชา+อ่อนแรงแสดงว่าเป็น peripheral n lesion

---C6 C7 spinal nerve radiculopathy ให้ตรวจ spurling test
---Radial n palsy--> saturday night palsy--> weak all extensor muscle in forearm and +/- Triceps +/- Supinator
---Posterior interosseous n syndrome, unlikely due to pure motor n
Rx.
Splint / Maintain position / ROM / Prevent m atrophy and joint stiff
NSAIDs
Vit B1-6-12
Neurapraxia observe 2-8 weeks ; 80-90% of cases มี recovery
