ขอขอบคุณทุกความเห็น ตอบมาได้ดีครับ ผมก็เพิ่งเคยเจอ case ได้ refer ไปแล้ว รพศ. รับ admit ถ้าได้ข้อมูลจะ progress ให้ทราบอีกครั้ง แต่เท่าที่ค้นดูมีดังนี้DDx. splenic calcification
-Cyst wall of calcification
-Granulomas, tuberulosis, histoplasmosis, brucellosis
-Sarcoidosis, toxoplasmosis, PCP, fungus abscess
-Phlebolitis or ateriovenous fistula
-Hematoma
-Splenic artery calcification
-Dermoid
-Epidermoid
-Splenic artery aneurysm
-Lymphangioma
-Hemangioma
-Hemosiderosis
-Subcapsular infarction or hematoma
-Thorotrast spleen
case นี้ นี่ยากอีกแล้ว นะ
ตอบลบเป็น case ไม่รู้ immune status host มาด้วย subacute fever with spleenomegaly ประวัติกับ PE มี่แค่นี้ ก็น฿กถึง กลุ่ม infection อันดับแรก , malignancy ก้ต้องเป็น อีก กลุ่ม ที่ต้งคิด ถึง กับอายุขนาดนี้
lab ,มีแต่ film เป็น multiple varing size of calcified nodule อยู่ที่ spleen
ultrasound มี multiple hyperechogenic nodule with aqustic shadow บริเวณ spleen
differntial infection ที่ทำแบบนี้ได้ก็น่าจะ common ก็ TB
อื่นๆที่ hostแย่ๆก็ fungal infection ที่ชอบเอามาออกสอน filmอาจารย์ GI ของรามาจำชื่อไม่ได้แย้ว ก็ histoplasmosis
จริงๆมี infection อีกหลายตัวเลยที่ ทำให้เกิดได้ เช่นถ้ามีประวัติ เลี้ยงแพะ กินนมแพะดิบก็ brucellosis
ส่วน malignant metastasis ก็คงอยู่ใน diff ของ calcified spleen แต่ไม่เคยเห็น case ง่ะ ไม่รู้จิงๆ
ก็เอาเท่าที่รู้มั่วๆนี่และกันนะ
case splenic calcification ไม่เคยเจอเหมือนกัน ต้องใช้ อาจารย์ google ช่วย ได้ความว่า
