149. Sensorineural hearing loss/Weber and Rinne test

หญิง 28 ปี หูอื้อ มีเสียงดังในหูซ้าย 1 เดือน เป็นหลังจากตกเรือแล้วหูซ้ายมีน้ำเข้า แต่ไม่มีหูอักเสบ PE: Ear canal and tympanic membrane intact both side, Decrease hearing Lt. side, Weber ฟังชัดข้างขวามากกว่า, Rinne test: Air ดีกว่า bone ทั้ง 2 ข้าง เป็นอะไรได้บ้างครับ
Webwer test :ได้ยินชัดข้างขวามากกว่าซ้าย(ซึ่งข้างซ้ายเป็นข้างที่มีปัญหา) บ่งบอกว่าหูข้างซ้ายมี sensorineural hearing loss ส่วน Rinne test : Air ดีกว่า bone conduction ทั้งสองข้าง เรียกว่ามี positive Rinne test บ่งบอกว่าไม่มี conductive hearing loss ของหูทั้งสองข้าง และช่วยยืนยันว่าข้างซ้ายมี sensorineural hearing loss ซึ่งในผู้ป่วยมีประวัติจากการบาดเจ็บที่เป็นไปได้คืออาจมีการกระทบกระเทือนต่อ inner ear หรือ cochlea หรือ cranial nerve 8 ก็ได้
Sensorineural หมายถึง Inner ear, vestibulocochlear nerve (Cranial nerve VIII) และ central processing centers of the brain
