ชาย 44 ปี Seropositive มี plan start ARV วันนี้ผลตรวจตับผิดปกติ ผู้ป่วยรับประทานยา Fluconazole, Cotrimoxazole, Clarithromycin, Phenytoin เนื่องจากเคยชัก, B1,6,12 CD4 เมื่อ 3 เดือนก่อน = 55 [9%] วันนี้ทำ U/S biliary system: negative study คิดถึงอะไร ทำอย่างไรต่อครับ [ 3 wks ก่อน SGOT 96, SGPT 135, HBsAg and AntiHCV: negative] •Elevated liver enzyme in human immunodeficiency viral infection cause: Alcohol consumption, co-infection with other viral diseases, such as HBV, HCV infection. Drug, antiretroviral drug toxicity or the HIV infection itself, opportunistic infection eg:CVM, Mycobacterium tuberculosis, Mycobacterium avium intracellulare, Cryptococcus neoformans, unknow
•การรักษาคือตรวจวินิจฉัยและแก้ที่สาเหตุ เช่นหยุดดื่มสุรา, หยุดยาที่สงสัย, รักษาการติดเชื้อที่เป็นสาเหตุ ถ้าหาสาเหตุไม่พบหรือ liver enzyme ไม่ดีขึ้นคงต้อง W/U เพิ่มรวมถึงอาจต้องทำ liver biopsy
